Insurance & Payment Options for Behavioral Health Services
At Life Coaching 4 Kids Center, we want families to understand their insurance coverage and payment options before beginning services.
We accept several AHCCCS health plans, select commercial insurance plans, and may offer self-pay or sliding-scale options for qualifying families. Because coverage and authorization requirements vary, our team can help you verify your benefits and understand what may be required before your child’s first appointment.
We Accept AHCCCS
Life Coaching 4 Kids Center serves Arizona families enrolled in AHCCCS (Arizona Health Care Cost Containment System).
We are currently in-network with several AHCCCS health plans, including:
- Arizona Complete Health
- Banner University Family Care
- Health Choice
- UnitedHealthcare Community Plan
- Care1st Health Plan Arizona
Insurance participation can change, so we recommend contacting our office before scheduling services to confirm that we are currently in-network with your specific plan.
Commercial Insurance
Life Coaching 4 Kids Center also accepts select commercial insurance plans.
Coverage for behavioral health services varies by insurance company and individual plan. Some plans may also require a referral, prior authorization, or other documentation before services can begin.
Our staff can help verify your benefits and explain any known coverage requirements before your intake appointment.
Self-Pay & Sliding-Scale Options
If your family does not have insurance or prefers to pay directly for services, please contact our office to discuss available self-pay options.
Some services may also be available on a sliding-scale basis for qualifying families, depending on household income, eligibility, and availability.
Our team will work with you to review the options that may be available for your family.
What Will My First Visit Cost?
The amount you may owe depends on your insurance plan, benefits, deductibles, copays, authorizations, and the services your child receives.
Before services begin, our team can help:
- Verify your insurance benefits
- Explain applicable copays or out-of-pocket costs
- Review authorization requirements
- Answer billing questions
- Discuss available payment options when needed
Our goal is to provide clear information before services begin so you can make informed decisions about your child’s care.
